Towards the back of the Prospectus (around page 215) there is a Priority Offer Application form.

The next page provides a guide to filling out the form.

Bpay isn't an option as far as I'm aware, only cheque or bank draft. I used a bank draft and didn't have any problems.

You then just mail (or hand deliver) the form and cheque / bank draft to the address provided at the bottom of the "Your Guide to the Application Form" page.

Since you mail the form to Link Market Services I guess they are handling / helping out with the offer.
