I'll make it less confusing -
1. Tick the small box on the right at the top appointing the chair
2. Tick all 16 resolutions Against in the middle row of boxes
3. Mail/Fax it in well before the 24th Feb

As moloch said, if you leave it blank - the chair votes their way. This is the only surefire way of ensuring your voting intentions are registered. If an "Act of God" somehow kept you from that meeting, I'm sure a lot of people would not be impressed.
