Having project managed several large IT projects, there's no excuse for missing a go live date, especially for a reason as basic as this. The approval should have been submitted much earlier than 28 Sept.
Missed deadlines do nothing for investor and or user confidence and rightly or wrongly it gives the impression of incompetence.
The old saying always stays true - under promise and over deliver. It's a mantra to live by in project management.
