It's generally not that healthy people develop health conditions (like black lung) from coal power-borne pollution, it's that old/young/sick people - who may already be at risk of death from cardiovascular events and the like - are pushed over the event threshold by pollution, often in combination with extreme weather. It's well established in enviromental health literature.
