4. if their endpoint was at week 60, -7 vs -10 at week 26, would the results have statistically significant?

The significance would depend on the value in the placebo group and the width of the error bars. So the drop from -10 to -7.5 doesn't matter quite so much as the consistency of the response and the difference from any placebo group. So if the worsening of scores at week 60 represents the diminishing placebo effect, that's actually a good thing since the rest represents a real improvement from NTcell - i.e if at 1 year the treated patients had an improvement in their scores of -7, and any placebo group was 0 or +1, that could well represent a significant improvement.
