No need to apologise about focussing on understanding the list - that is what this thread is for.
ML may well also have all the shares that he owns held in a trust, he the listed trustee for this trust, and there may be multiple other trustees too (which we can't see as only one trustee is required to be listed).
He could also hold amounts in his personal name too small to show on the top 20 list.
