1. I haven't read the whole thing but I doubt it was from the US study as the submission date of the paper is before US results were announced.

3. yes blind
4. yes testing happens as enrolment progresses
5. most likely with the exisiting patient pool that investigators have. would be surprised if they need to advertise.
