I don't think Iran will use them if they had nukes.
But a problem with proliferation is if the govt becomes less stable the nukes might be up for grabs by any radical group. The same concern after the Soviet Union collapsed. There is also the lingering worry that they could supply guerilla groups in the area to do their dirty work.
