I suggest that everyone stop focusing on the difference in the results between the USA and Australia and start focusing on the patient selection and/or scoring. Given the numbers in the trials (too large to just be random noise) these results look to be due to a difference in the patients, or in they way they are assessed.
I suspect that the problem is in the way the patients were assessed in the USA.
