I was on BHP site 10 years ago when a 19 year old was run over by a haul pack truck and Tyre fitter crushed by Tyre within 10days. Both were clearly dead but not pronounced dead until taken to the coroner off site (I think off site). We were shut down for 24hours after the first death and then 3 weeks after the 2nd death.
