My first week doing FIFO I got the knock on my donga from the near-burnout 2 doors down who came to tell me about the noise my chair made rolling across the floor at 10pm. Big beard, a twitchy voice and indirect gaze told me FIFO had a definite drag effect on people over time. He'd been doing it just 2 years straight.
So, it wasn't surprising to see people "disappeared" all the time. "Where's Bob?". Gone. "Well, he was here before lunch!". Mgmt. didn't waste 30 seconds getting people off site for the slightest issue. It seems unlikely this incident just blew up from nowhere so, yeah, it's very odd that no one reported the escalation.
