You make some good points .... this process is so ridiculously complicated to the point of incredulity. I had assumed that the LCD was determined by an independent board that made some sort of cost benefit analysis a little bit like the FDA. However it seems that LCD inclusion is determined by the Medicare Administrative Contractor (Novatis) who are contracted by Medicare to run the scheme in a certain geographic area. Novatis seems to have the New York Area and Texas Area covered. So if this ticks a key criteria off their list then it has to be unambiguously good. I did wonder why the share price reaction was strong to news that seemed to prove what we already knew. It is not clear to me at this point if you need to get approved coverage from each contractor for each area or if they all input on to a national list, the clue maybe in the local part of the LCD.
