A typical response from someone that has never dealt with forms or Govt or Semi-Govt institutions.
The form has a standard name and serves the dual
purpose of an appointment, or resignation or both.
I've almost finished my hunt through the 2018 accounts and seeing this as a prelude to Brewer's ousting I'll dig a little further before commenting.
