I am not a doctor however I do have an overactive immune system which often causes this kind of problem (obviously less severe), so I have some experience with it. I am not understanding what exactly it is that you do not understand but I will try to explain in my own way.
When something foreign, like a virus, enters out bodies, our body tries to expel that foreign thing. Perhaps it is a pollen, dust, or a virus. Our bodies produce fluids like mucous and phlegm and tears, and through sneezing and coughing and watery eyes, our bodies try to expel that foreign thing outside the body through our eyes, nose, mouth. Right? Well, that’s our immune system working efficiently. Doing what it’s supposed to do.
Well, sometimes, for some people, the immune response goes overboard. We breathe in some pollen (or a virus etc) and our bodies react in an extreme way by firing nukes (lots and lots of phlegm etc) at it, so much that we can’t breathe. And the process gets into a cycle where nukes are being fired left, right, and centre, way too much for what is needed, damaging the host as well (the person’s healthy body becomes damaged). In the case of covid-19, the air is reduced so much that blood is not oxygenated and then that causes problems for the heart and other organs, leading to organ failure....all from not get enough air.
So, what is needed is that we want to tone down the immune system to behave in a more controlled and calm way. We want the immune system to fight the fierce invader but not so brutally that the healthy body (like the lungs) gets damaged. If we used steroids, then we would switch OFF the immune system for a few days, and that could be a problem because the virus then runs wild unchecked. So, maybe we don’t want that, we want to just tone it down. Using antibiotics works for some invaders, but the covid-19 invader is a nasty one which doesn’t respond. So, toning the immune system down a bit is really tricky and takes great skill. We need the body to fight...but not so hard that it damages healthy tissue. By using stem cells, the objective (I think, and please correct me if I am wrong) is to reset the way the system works, so in this case it means resetting the immune system to react in a healthy way, in the way it is intended to work.
Anyway, that’s the way I see it.
    I hope I haven’t confused you more.
