I believe DGR will send you a personalised from which shows you no of shares. A slot for the $ value of 1/4 of that as the entitled amount at 0.037 and another slot for over subscriptions. (Or maybe one slot for the total).

It will also have detials of how you can pay for it one of which is the Bpay with BSB & Acct no. (cheque is also poss). That, probably A4 cover sheet with you name address (and HIN elsewhere), will be with the prospectus. I'm glad you somewhat happier now and I expect you average buying cost after this will be well below mine FWIW.
