I'm in NZ and got a letter sent to me with the application form, as above, but with my shareholding details on it. I filled out the form and emailed it to [email protected] as per the advice on the form. I then did the bank transfer as per the advice as well. I have 7000 shares so figured it was only worth sending in NZ$2000 as that would exceed the maximum I am likely to get, since it is such a small raising.
