Page 13 of the booklet talks about the options,
if using bpay you don't have to submit a form, just use the bpay details that are on your application form.

The online form I assume only needs to be done if you somehow didn't recieve a form in the mail. because it asks for your details to verify that you actually are a holder. But since you already received the form in the mail you just pay the amount you want with your individaul bpay details on your form.

It was a bit confusing for me to for a bit
