    Greetings,

    I have only today received the Entitlement and Acceptance Form together with the Prospectus.

    The Prospectus, Clauses 4.3 and 4.4 allow for Payment "via Credit Card by following the instructions set out on the Entitlement and Acceptance Form".

    No such instructions exist on the Form that I have received.

    Does anyone else have the same problem.
