Yes, you will receive an application letter in the mail, presumably from ComputerShare (so make sure your address details there are correct).

You fill it out and send it back with your money (you have until 11th Jan 2021 to do this). Then on 18th Jan the number of shares that your trading platform tells you you have will increase by that amount.
