I agree, if the trial was run well there's no reason to think there wasn't proper blinding.
There wouldnt be one doctor treating all these patients, it's going to be multiple teams in multiple hospitals rotating around.
There's no way they'd be able to deduce what arm they were in. Especially if the treatment offered only a small benefit.
