Unfair to whom, dp? The point is we won't know it works until it is compared against placebo. If you mean, unfair to the patients who receive placebo, that's a fair point, but should the difference in outcomes be obvious and marked the trial will usually get cut short and the placebo patients then receive the drug treatment. Which no-one would have got without doing the trial. So still a win-win.
