If A lends the shares to B (shorter) who sells the shares to C. C owns the shares. A owns the rights to those shares which B has to return someday. When A wants to exercise the rights from B, then B has to buy back the shares, at whatever price, on the market from other shareholders (C, D etc) to return to A.
