Okay, just contacted CTM yes you can get the form from CTM or Advanced Share Registry it's the same form. You have the option of EFT monies direct to CTM account (account details on the supplied form) but the Notice of Exercise goes to Advanced Share Registry.
They do have up to 15 business days but stated today that they are issuing current requests this Thursday & they will endeavour in future to convert requests on a weekly basis as to date they have been busy with the SS.
