Motty you don't need to do any forms. I am in Indonesia and can do it all via Bpay.
I just completed the same with HST via Bpay and PEN has the same facility with this one. When the entitlement comes it will have your specific Bpay details on it.
It could not be easier and I wait patiently for mine. Without question I will be in this boots and all.
