ASIC has a complaints section on its website - just google ASIC company complaints.
I would say you're a shareholder, that the company was delisted from the ASX and the Board has failed to prepare and send financial statements to shareholders or call an AGM since 2019. it doesn't need to be elaborate, just as long as you make those points.
