I believe some PhD's (philosophy doctorate) believe they are the real doctors and GP's/medicos are the ones with the honorary (given they do a degree but not typically a PhD. I guess it depends on how you look at it.
Some use it, some don't. I can use the title but rarely do. To be fair I'm just an Osteopath so no Phd or medical doctor degree.
