You can apply for up to $15000 (ie $0 - $15000) at a price of $1.40 each.

When LNC have all applications upon the close date, they will (most likely) scale back your application if need be.

Ie - if shareholders apply for $40million worth of shares, you will get 50% of what you applied for.

The additional information they refer to would be closing dates for applications, and the information enclosed with the form they will send you with a summary of the SPP on it.
