    My friends,

    The stars are magnificent. The Milky Way is glorious, a redemption, an inspiration. To see the rings of Saturn, the moons of Jupiter, Magellanic clouds, and to just watch the Celestial sphere’s eternal pirouette. Ah, back now, back on grid. After contemplating the infinitely large, back now to contemplate and focus on the microscopic. And at the microscopic level, as Richard Feynman said, "there is plenty of room at the bottom". Modern medicine is about the micro-universe.

    Today, Tiresias would just like to say he’s back on-grid, and to say hello to his friends. Whilst off-grid, looking at the celestial sphere, his thoughts traversed the expanses of space and the time. He thought a lot about the history of medicine and the rise of modern medicine, and the exponential explosion of medical technology in the second half of the 20th Century. Though Tiresias has written on the history of modern medicine, in relation to the place of In-vivo Real-Time Digital Pathology, it is always worth reminding ourselves of where we are in this whirligig of ever accelerating technological explosion. As Tiresias has said before, modern medicine begins in the 16th and 17th century with Anatomy, the first Ex-Vivo morbid post-mortem Macro Imaging. The invention of the first light microscope leading to the discovery of cells and cell theory; and the discovery of germs, and germ theory, leading to immunization and antibiotics.

    In these early centuries of modern medicine, cancer was not a major issue. Cancer was there, but illness and death due to cancer was uncommon. Most people did not live long enough to develop cancer. The killers were infectious diseases. Most people born, died in the first year of life of infections. The world has indeed changed and continues to change. Though we are in the midst of a pandemic, this pandemic is not the Black Death. Tiresias is in no doubt that the technology available can handle this. Cancer however is omnipresent, increasing, and cannot be beaten with isolation and vaccination. Cancer is "The Emperor of all Maladies". And this is the real health battle, the real war. And this is why we are here. Optiscan Imaging to Confocal Laser in the Endomicroscopy is a paradigm shift in cancer diagnosis and cancer treatment. The majority does not understand and hasn't even heard the news. Mention In-Vivo Real-Time Digital pathology in the clinic and in the operating theatre to most doctors and you will get a blank look. But that blank look is the opportunity. Tiresias sees a cancer treatment and diagnosis epiphany, and soon.
