What a bad disclosure document.
How can a company "Cease" to be a substantial holder when all they do is "Purchase" shares ?
My guess is that the Form used should have been a "Change" form.

IMHO, Pauline needs a quick lesson in how to complete the forms .....
Just give the ASX a call Pauline, please.
ASX should call her if she does not call them.
