"why is this never discussed?"
That is a very good question and I can only think it is not discussed due to PC gone mad. That is, the risk of offending the obese population is considered a bigger problem than prioritisation and focusing of the public health response.
If that is the case, it is a pathetic indictment of governments around the world!
