We know the FDA requested modifications to Paradigm’s adrenal screening and mitigation plan. We know Paradigm amended its clinical trial protocol, including all the FDA’s requests, and responded to the FDA.

It is most likely to be some further issue with the proposed changes. The most obvious answer is usually the most likely people (for all the fear and optimism above).
