I believe he's referred to himself as a janitor for a health facility in the past. He's just never corrected anyone who's referenced him as possibly being a doctor and he rides it. He's even gone as far as scribbling his name on a post-it and taking a photo of someone else's desk with a stethoscope on it. All very sly stuff.

Most doctors I know don't have time to scratch themselves let alone post 16 times in 3 hours on a Tuesday. If he was cleaning an office I'm guessing he's an evening worker so he has all day to brood, most days. As someone who manages the equivalent of "janitorial" staff at a medium sized health care facility I can confirm that they do love their brooding.
