I cannot take any responsibility for the accuracy of information below, but offer in good faith.
CBA was ‘privatised’ in three separate tranches, in 1991. 1993 & 1996. Following the initial share offer in 1991, the CBA registry was maintained by Coopers & Lybrand.
It may be relevant to mention that the first tranche of shares were listed on the ASX on 12 September 1991, so in FY 1991/92. I can also tell you that the 1991 IPO price was $5.40.
The second (1993) tranche was referred to as the ‘Commonwealth Bank Secondary Share Offer’, and the price was officially set at $9.60, but there was a ‘Discount Price’ of $9.35 which applied to the “first $25,000 of any application”.
The third tranche was a little more complicated, in that applicants applied for “Instalment Receipts” for CBA shares with a $6.00 initial payment ‘on allocation’ (allocated in July 1996), followed by ‘Final Instalment’ of $4.45, with payment due by 14 November 1997. Thus, the ‘Total Issue Price’ was $10.45, and from what I could find, the shares were certificated on 21 November 1997. Coopers & Lybrand remained the responsible share registry at this point.
It may help if I mention that the dividend paid on 30 September 1997 did appear to apply to (existing holdings, plus) the (new) IRs which had, at that time, not been fully paid off, but of course, carried a payment obligation.
Regarding the share registry details, it looks like C & L continued through until at least 1998. After which it seems to have gone to ‘ASX Perpetual Registrars Limited’ (now Perpetual Ltd), and they were the responsible registry past the time you mentioned in 2002.
