Hi hamman, great question.
The single most important recommendation by my physician was physical training to develop a strong core. The reason for this is that the joints are supported and stabilised by the muscle, ligament and tendon structures around them, and that strengthening that structure would be the most effective intervention for slowing progress of the condition. I already have a pretty strong core because that is the nature of my sport, and I wonder if that is why I didn't really notice things until my OA had become severe. In any case I've added further core work to my rotation (eg. check out Turkish getups on YouTube).
After that, the options were pain killers, then a spinal fusion. The spinal fusion does not really fit into my plans, pentosan is a much better option.
