But, it's very telling...
No one here is focussing on blame, which is why your post is bizarre in its irrelevance.
But, the reality of the issue still exists.
The fact that you see that blame can be attributed means you do understand that the problems exist.
The problems which exist are relevant, regardless of who gets the blame, and no one here is particularly fussed about blaming anyone.
Blaming is largely irrelevant, but, if someone displays a very consistent track record of causing problems, whether or not you're hung up on blaming them, it makes sense to assume that they're probably going to cause problems (or turn up late or fail to deliver or lie or whatever it is they're doing wrong, or indeed, right).
When the carrot is continually dangled but never put within reach, when promises are empty, when words are lies, when deadlines and not met, when results are not delivered, these are still actual problems which should be understood and responded to appropriately, regardless of whether or not you care about blaming anyone.
Blame whoever you like, or don't blame anyone, but the "failures in timing, analysis, risk & uncertainty assessment, etc etc…." (your quote) are still realities of AGY.
