Awful performance by both management (responsible for execution) and the board (responsible for relevant & timely reporting). Missed all relevant guidance by a big margin, yet failed to inform until "the death".
Needs a responsibility re-fresh for both board and management - particularly board - or put the For Sale sign out!
Something needed to arrest the spiraling decline.
