Let me throw in one from left field :

Peyronie disease - Peyronie's disease is where plaques (segments of flat collagen scar tissue) form under the skin of the penis. These plaques can cause the penis to bend or become indented during erections. The plaques can often be felt through the skin and can be painful. The phrae "bent like a one string fiddle" comes to mind.

Current treatments are either surgical or a collagen solublising enzyme via injection (both are purported to be painful).

As "amusing" as this may sound, the indication is that Peyronie's afflicts about 4 out of 100 men between the ages of 40 and 70. A cream that removes or alleviates scar tissue by topical application sounds like an absolute winner. With a potential market of a few hundred million people.
