It's not about been in guidelines, it's about the advisory committee understanding the science and the benefit to the insurer.

This can also be outsourced to research providers who provide a recommendation using their expert knowledge and passing that onto the committee.

thanks, understood that it is their policy not nccn that matters. Was confused as your list (below) seemed to indicate they would update the policy in 2025, I have just misunderstood. All good though, thanks for the extra detail.
