I've been doing some research as I've had concerns about whether 327 gel is the quick commercialisation and income stream we need.
Around 30% of DFUs turn into DFIs. These respond well to specific antibiotics with 57-69% success rates. Newer antibiotics can see as high as a 90% success rate in some trials. One of the main challenges that leads to high amputation rates is the usage of inappropriate antibiotics. Lack of awareness, supply and financial access to the specific antibiotics may hold responsibility for this. Recce 327 in phase 2 trial had <50% cure rate with improvement in most cases. This raises a few issues:
Is this really an unmet need? Even though it works well-ish, it doesn't seem to offer anything drastically better than current antibiotics and care? Will hospitals in Indonesia have the awareness, finances and will to purchase 327? Knowing the indonesian governments stance on pharmaceuticals 327 will be sold at bargain basement price. How much will Etana take as their cut?
How much income is this really going to generate???
