It's very common for companies to fax a copy of their templated document.. templated meaning that it has fields such as dates and name and number of shares to be field out for each individual shareholder. The fax sent to extract is not specifically directed to an actual shareholder.. thus the "insert date here" fields.
So don't worry. your mailed copy of the document is the one that counts and that will be filled out properly.
more importantly, we should be worrying about how much exactly we are going to get ripped off by Taurus!
For me, the offer price is a joke, not the faxed document.
