Your problem is T+3. ALK traded on 6th Ex Entitlement, meaning you had no entitlement to participate in the entitlement offer to buy new shares for those shares.

If you look at your contract note you will find -
1. a notation Ex Ent as the basis of trade.
2. the settlement date was overnight on 9/3/12 resulting in the registration of your shares missing the closing date as you did not legally own them at that time.

Remember for the future that the important date is not the date of your trade but the settlement date for the trade.
