Actually I think it's quite productive to speculate why Xanadu failed considering it took 2 - 3 years of time from clinicians, patients, and the company as well as a few million dollars at least. Again, if 10mg a day managed to significantly lower intracellular cortisol, judging from effects on ACTH, androstenedione, etc, then why don't we see any differences in cognition?

Even after 2 years, the team still haven't explained why 10mg has failed, and it's because they don't know - which is a bit worrying to anyone invested in the company. 10 mg is supposed to cause at least 50% inhibition of 11BHSD1 and in animal models those amounts of inhibition were adequate, so to me it's troubling that a drug did 'everything it was supposed to do'... except hit all the primary and secondary endpoints.

Again, I'm still holding out on the promise and potential, but let's be honest with ourselves and recognise that the case is still very much open as to whether Xanamem will work.
