I think if we were going to introduce psychology to the thread then this would be a case of either conscious or unconscious incompetence.

Seems some rough calculations done here by slampoil match up fairly well with my own $12mill back of the envelope calcs.

But that may be just one of the unknown things we know we don't know that we know.
