Some have in hub gears, others run traditional chain and sprocket systems with either just a rear cluster or front and rear clusters. Some have motors mounted at the wheel, others at the pedal crank (mid mounted). For the weight of the motor on an average e-bike and for the given weight of battery capacity that is reasonable to carry, gears are essential to deliver desired torque at full range of speeds - so gears stay.
