That notice is to exercise options you muppet
It has nothing to do with the rights issue unless you want to exercise your options (by 5.11.21) so that the shares you get for your options are registered in your name by the "record date" (8.11.21) which then means you are then entitled to 1:6 new shares in the rights issue
No form, no money by 5.11.21 means you are not a shareholder by the record date & can't participate
Read the subject line of the letter (it is in bold type so you don't get it wrong) It says "Notification to option holders..." & the first words in the second last paragraph you refer to state "If you wish to exercise your options...."
Take some time next time to read what is put in front of you
