Ab Initio's SMI is a way more advanced spring powered-tech (read costly$$$) that sprays mist like the name says - much more slowly and, notably, without a propellant. This has eco-friendly connotations and sounds healthier for the lungs.
We are using a regular low cost, mass produced pMDI. It's a faster and more cost-effective route to FDA approval. They are tried and tested, smaller than a SMI, and easier to use; that is, patients don't need help from a caregiver, which is important when a patient requires immediate relief.
SMI's do have merit for late stage cancer sufferers when motor or cognitive functions decline in a hospital setting with caregivers, but overall pMDI's are a more reliable default for independent use during acute unpredictable pain episodes which is the nature of breakthrough cancer pain.
It's a good question, however. Who knows IRX 211 could ultimately be marketed in both forms?
