I have had the odd employee tell me "not my fault", "outside of my control","didn't see that coming" "no one expected that to happen".
Everyone gets a few free passes of course because stuff does happen. However, once you hit 3,4,5 not my faults. Good bye we will find someone who can be ACCOUNTABLE & RESPONSIBLE because that's why they get paid.
If you don't hold people accountable and responsible then it's a slippery slope.
