Good points you have made Southoz.
Also if the world's population keeps doubling in half the time it previously took to double then the priority is not going to be on subsidising massively expensive treatments be it for cancer of any other disease.
So the shift as you say is likely to be considering cost effectiveness as one of the main parameters as well as tolerability and ease of use rather than solely subsidising based on efficacy.
