Hi all,

I am looking at completing Form B for the share deal, and on the back on Step 2 it asks for you to complete a AML/CTF Investor Identification Form.  I rang the enquiry desk and they said they would send such a form out, but they have not. I can ring and ask again, but I am wondering what others have done in relation to this? Pretty poor form to have not included it, either initially or in the recent update....
