Frankly, you'd have to wonder why Mr Butler was on the wall in the first place. I don't care how much you're paid if you think its dangerous then why would you get up on the wall. The same happened at Grange Resources in 2017, I don't remember GRR being fined for that accident.
It was an accident, if you work in mines its inherently dangerous, I worked in NSW coal mines as a mining engineer cadet for BHP in the 1970's and its your own responsibility to take care of your own well being. If you see a loose rock overhead, I wouldn't walk under even if I was told to do so. There is also an element of self responsibility for your own safety.
Tragic case there is no doubt about it, I saw lots of accidents during those times, not all are managements fault.
